    I know this part of the journey well because I’ve walked it myself for decades and in several different capacities. Step 3 of life-care planning calls upon the care recipient to honor those who give selflessly on their behalf: care partners and the entire care-partner team.

    In recent months, we’ve been diving into the life-care planning steps highlighted in my new book, “Because I Love You: My Book About Me.” So far, we’ve discussed:

    As you’ve likely guessed, the goal is to plan A LIFE WORTH LIVING for our elders and their care partners. There are many moving parts to a great life-care plan. Some of them can be quite complex, monotonous and overwhelming – gathering and organizing financial and legal documents; and coordinating care, doctor visits and end-of-life plans. Others call for personal reflection and documenting life’s interests and wishes, essentially drawing a map for the best quality care.

    Honoring Our Care Partners: A Personalized Approach to Life-Care Planning 

    As the primary care partner for my husband, Brian, who lives with Alzheimer’s and vascular dementia, I’ve experienced the emotional highs and lows. The challenges and triumphs that come with being a caregiver are familiar as old friends. 

    I know what it’s like to feel underappreciated and that my needs are secondary. That’s part of the sacrifice. But it doesn’t have to be that way.

    Care partners must be recognized as a critical part of any caregiving journey because, without them, everything falls apart. We’re the sympathetic hand holders. This step in the process is all about recognizing and supporting the incredible care partners who devote their time, energy and love to ensuring elder loved ones receive the best possible care.

    As care partners, we wear many hats. We’re caregivers, organizers, advocates, cooks, housekeepers, drivers, and sometimes even medical coordinators. We pour ourselves into making sure our loved ones are safe, comfortable and well cared for. 

    But who takes care of us?

    I’ve seen it too many times: care partners who are stretched so thin, running on empty because they’re trying to do everything on their own. It’s easy to put your own needs on the back burner. But if we’re not caring for ourselves, we can’t give the best care to our loved ones.

    By acknowledging the incredible work you do, we can begin to build a support system for caregiving in a sustainable, happy and fulfilling way.

    If you’re like me, asking for help doesn’t always come easily. Being a care partner is one of the hardest jobs in the world – and the majority of us do it without pay. Many of us also work full-time jobs, raise families and balance countless other responsibilities on top of caregiving.

    One of the most important aspects of life-care planning is making sure care partners have access to support, time off and resources. This isn’t a luxury – it’s a necessity. you can’t pour from an empty cup. You need breaks. You need self-care. You need to feel appreciated.

    Avoiding Frustration & Overwhelm: It’s OK to Feel What You Feel

    If you’ve been a care partner for any length of time, being overwhelmed is a way of life. Caregiving is emotionally challenging, and it’s OK to acknowledge that. In fact, it’s important to.

    In this step of the life-care planning process, I encourage you to take a step back and reflect on your feelings. It’s OK to admit that this role is hard. It’s OK to feel tired, frustrated or even angry at times. What’s not OK is ignoring those feelings and pushing through without taking time to process them.

This isn’t just about making a plan for the care recipient – it’s about making sure you are supported, too. There are thought-provoking questions designed to help you express your frustrations, fears and needs along the uncertain path.

      Don’t hesitate to interview prospective Patient Advocates to gauge their experience and expertise. Patient Advocacy isn’t a one-size-fits-all calling. You need someone to handle specific elements of elder care – from medical knowledge to organizational skills – for your loved one and the care-partner team. 

      Ask about their background, approach to advocacy, and how they’ve helped other families in similar situations.

      4. Evaluate Communication Skills

      Communication is crucial. Plain and simple, a Patient Advocate can’t do their job if they can’t nurture effective communication among all members of the care-partner team – both professional and personal. 

      Choose a Patient Advocate who listens carefully, communicates clearly and promptly, and is responsive to your needs and concerns.

      Signs Your Elder Loved One Isn’t Taking Their Medications (& Tips for Getting Back on Track)

        I know you’re a dedicated and compassionate care partner. You try so hard to provide a safe, caring and nurturing environment, but it can be mind-numbingly difficult to manage and monitor EVERYTHING. Never is that more apparent than when your elder loved one isn’t taking their medications.

        Sometimes it’s glaringly apparent. Perhaps your mom is acting disoriented because she’s been missing a dementia-related medication. Or your aging brother’s blood pressure and cholesterol numbers were off the charts during their latest doctor visit. 

        Other times, the signs aren’t so easily noticed. Failure to adhere to prescriptions might stem from the common struggle for older adults simply to remember their medication list. It could also be outright avoidance. 

        Whether it’s due to forgetfulness, side effects or other reasons, nonadherence can have serious consequences on their health. In this blog, we’ll share some signs that may indicate your loved one isn’t taking their medications and offer practical tips to help them get back on track.

        Recognizing the Signs Your Loved One is Avoiding or Missing Medications & Treatments

        From health declines to evasive behavior, these are some things both primary and professional care partners should look out for: 

        1. Sudden Health Decline or Recurrence of Symptoms

        One of the clearest indicators that your elder loved one might not be taking their medications as prescribed is a sudden decline in health. Sometimes, this may come in the form of a recurrence of symptoms that had previously been under control. For instance, if their blood pressure spikes or their diabetes symptoms return, it’s worth investigating whether they are following their medication regimen.

        It’s important while assessing declines in health to recognize normal vs. abnormal signs of the natural aging process. If you’re unsure, contact a primary care provider to help you make such determinations.

        2. Unusual Behavior or Mood Changes 

        Medications play a crucial role in managing not just physical health, but also mental health. Millions of older adults take prescriptions from common ailments like anxiety to specialized treatments for more severe disorders like Alzheimer’s and other types of dementia. 

        Be on the lookout for unexplained mood swings, increased irritability, other behavioral changes, or regressions in memory and cognition. It could be a sign that they are not taking their medications properly – or not at all.

        3. Confusion or Memory Problems 

        On a similar note, older adults with cognitive issues may forget to take their medications. But individuals don’t have to be living with dementia to experience forgetful moments commonly associated with age. 

        If your loved one seems more confused than usual or is having trouble remembering daily tasks, they may also be forgetting their prescriptions.

        4. Full Medication Bottles 

        Keep an eye on their medication bottles. If your loved one is relatively aware cognitively, you may be inclined to give them space and trust that they’re following the doctor’s recommendations. But it may be wise to “trust, but verify.”

        You may encounter a bottle that should be nearly empty but still looks full. Putting 2 and 2 together, this would be an obvious indicator that doses are being missed regularly. Similarly, if there are no signs of refills when there should be, it’s time to investigate further.

        5. Resistance to Taking Medications 

        Your loved one may become defensive or evasive when it’s time to take their medication. They may ask to be left alone during advised medication administration times. This could be a sign they’re avoiding having to take them.

        This behavior can stem from various concerns, such as unpleasant side effects or a lack of understanding about the importance of their medications.

        6. Unexplained Physical Symptoms 

        Pay attention to any unexplained physical symptoms, such as dizziness, fatigue or gastrointestinal issues. These can sometimes be linked to missed doses or improper medication management.

        Tips to Help Your Loved One Follow Their Prescription Medication Regimen

        If you’re thinking your elder loved one isn’t taking their medications, for any reason, here are some tips to get realigned.

        1. Open Communication 

        It’s amazing how many things in life can be fixed with open and honest communication. Begin by having an open and non-judgmental conversation with your loved one. Ask them if they have been taking their medications and listen to any concerns they may have. Explain your concern for negative health consequences if they continue to not take them.

        Begin from an area of trust. Even if they’ve been avoidant – or even somewhat “sneaky” – that doesn’t mean they’d automatically lie when confronted. They may even open up about their own concerns. 

        Understanding their perspective can help you address root causes.

        2. Address Side Effects

        Your loved one may have very reasonable and justifiable concerns about their prescription medications – even if they’re having difficulty expressing them. Many meds do indeed come with wide-ranging side effects that can make older adults feel icky or cause more serious medical consequences. 

        If your loved one is experiencing unpleasant side effects, they might be reluctant to take their medications. Discuss these side effects with their healthcare provider to explore alternative treatments or solutions that can mitigate these issues.

        3. Simplify the Medication Routine

        Complex medication schedules can be confusing. More than 20% of elders take five or more medications daily. Even the most aware people could easily forget a high volume of prescriptions, which must often be taken at different times in various ways.

        If it’s a matter of confusion, work with the individual’s healthcare provider(s) to simplify the regimen, if possible. This might involve switching to once-daily medications or using combination pills.

        (The next two sections address more simplification techniques.) 

        4. Use Medication Management Tools

        There are various tools available to help manage medications. Pill organizers, automated dispensers and medication reminder apps can make it easier for your loved one to remember to take their meds on time.

        5. Set Up a Reminder System 

        Help your loved one establish a routine that includes reminders for medication times. This could be as simple as setting alarms on their phone, downloading a scheduling app or using another type of reminder service. 

        Consistent timing can help make medication-taking a habit.

        6. Educate on the Importance of Adherence 

        Sometimes nonadherence stems from a lack of understanding about the importance of the medication. Don’t assume your elder knows what their meds help to improve – or the consequences of missing prescription times. 

        Perhaps they’re not skipping their meds entirely, but merely missing doses every now and again. Many prescriptions require the ongoing introduction of medicine over time to achieve a desired goal. Educate the individual – in a way that’s easy to understand – why it’s necessary and how they contribute to their overall health.

        7. Involve a Healthcare Professional

        Sometimes it’s better when an individual “hears it straight from the horse’s mouth,” so to speak. Schedule regular check-ins with their healthcare provider to monitor their medication regimen. A professional can reinforce the importance of adherence and make any necessary adjustments to their treatment plan.

        8. Provide Emotional Support

        Empathy and loving support can be all that’s needed. Nonadherence can sometimes be a cry for help or an indication of underlying emotional struggles. When a loved one isn’t taking their medications, they sometimes just wish to feel heard and understood. 

        So give them what they need! Offer them emotional support and reassurance, as you would anything else in the course of your caregiving duties.

        9. Encourage a Healthy Lifestyle 

        Beyond emotional support, we all need a little encouragement – a push from a devoted cheerleader. Even the most mentally and emotionally fit among us need it now and again. 

        A healthy lifestyle can complement their medication regimen and incentivize following it. Encourage a balanced diet, regular exercise and stress-management techniques to improve their overall well-being. This should all be part of a well-rounded life-care plan that factors EVERYTHING for A LIFE WORTH LIVING.

        Following a prescribed medication routine is crucial for improving and maintaining your loved one’s health and well-being. But sometimes it’s apparent the individual isn’t taking their medications. 

        Here we’ve covered some signs they may not be following their medical team’s recommendations, as well as some tips for getting back on track. By recognizing the signs and taking proactive steps to address it, we can help ensure elder individuals receive the full benefits of their prescribed treatments.

        Spotting the Signs of Elder Abuse: Let’s Protect Our Aging Loved Ones

          Very few things could make my blood boil more than elder abuse. In nearly three decades as a life-care planner, I’ve seen more than I care to share. It’s not always as obvious as it may seem, and it doesn’t always occur at the hands of strangers behind closed doors. So let’s protect our elder loved ones by recognizing these signs of elder abuse.

          As our loved ones age, they naturally become more vulnerable to various forms of abuse and neglect. They may not be strong or cognitively aware enough to defend themselves. This vulnerability makes it essential for care partners, family members and friends to be vigilant in recognizing the common calling cards of elder abuse. 

          Unfortunately, these ugly types of abuse are more prevalent than many realize: more than 10% of older people in the U.S. experiences some form of abuse. Sadly, only an estimated 1 in 24 cases are reported to authorities, according to the National Council on Aging. 

          Understanding the Signs of Elder Abuse: It’s More Than Meets the Eye

          Elder abuse can manifest in various forms including physical, emotional, sexual, neglect and financial exploitation. Let’s explore these in detail to recognize the warning signs to keep our elder loved ones safe.

          1. Physical Abuse: It’s Not Just Bruises

          Physical abuse involves inflicting bodily harm by hitting, beating, pushing, scratching or burning. Here are some telltale signs:

          • Unexplained bruises or wounds: These can appear on any part of the body, but are often hidden by clothing.
          • Bruises in various stages of healing: This suggests ongoing or repeated abuse.
          • Signs of restraint: Look for marks on wrists or ankles.

          But don’t be fooled. Physical elder abuse can be sneakier than that. Less obvious forms include the inappropriate use of medications to sedate or harm the individual, as well as confining them against their will. 

          Your loved one may appear unusually drowsy or frequently asleep, or they may show signs of trauma such as rocking back and forth. If they’re afraid or cognitively unable to verbally communicate, they may resort to physical gesturing and other non-verbal communication methods. It’s as if they’re sending out an S.O.S. that only a sharp-eyed detective — like you — can decode.

          2. Neglect: The Most Common, Yet Still Overlooked

          Neglect is the most common type of elder abuse. It occurs when a care partner fails to meet their basic needs. These can include medical, social, emotional and/or psychological factors. Signs of elder neglect can include:

          • Poor hygiene, dirty clothes, or a generally unkempt appearance: Your loved one is now looking unclean and disheveled.
          • Bedsores or pressure sores: Indicating prolonged periods without being moved and cared for properly.
          • Unsanitary living conditions: If their home looks like it’s been through a tornado, something’s wrong.

          Other signs of neglect can include malnutrition, dehydration, unexplained weight loss, and missed medical appointments or medications. When neglected, our loved ones may feel forgotten, so we must try extra hard as compassionate care partners to provide the attention and care they deserve.

          3. Emotional Abuse: The Invisible Wounds

          Emotional abuse encompasses behaviors that cause psychological harm, including humiliation, intimidation and verbal assaults. Unlike physical abuse, the signs can be much harder to spot:

          • Withdrawal from social interactions or activities they once enjoyed: Suddenly, your loved one prefers solitude over spending time with others.
          • Unexplained changes in mood or behavior: Tearfulness, irritability or agitation can be red flags.
          • Expressions of fear or anxiety around certain individuals: If Grandma’s suddenly afraid of the night nurse, something’s off.

          Less obvious forms of emotional abuse can include ignoring the individual, isolating them from friends or family, or forbidding them from participating in social activities. This form of elder abuse is like turning their life into a lonely island, and we need to build a rescue boat.

          4. Sexual Abuse: The Unspeakable Harm

          Sexual abuse involves any non-consensual sexual act or behavior. Warning signs can include:

          • Bruises or wounds on the breasts, inner thighs or genital areas: These can be obvious, but not always visible.
          • Unexplained sexually transmitted infections or injuries: These should raise immediate alarms.

          In addition to physical signs, other forms of sexual abuse can include forcing the elder to watch pornographic material or sex acts or making them undress against their will. It’s the stuff of nightmares, and we must stay vigilant to protect our loved ones.

          5. Financial Exploitation: The Silent Theft

          Financial exploitation involves the unauthorized use or theft of an aging individual’s money, property or assets. Signs of financial exploitation can include:

          • Unexplained withdrawals or transfers from bank accounts: Money seems to vanish into thin air.
          • Missing valuables or property: Items that should be there are suddenly gone.
          • Sudden changes in legal/financial documents: New wills, powers of attorney or other financial and legal documents that weren’t there before.

          Less obvious forms of financial abuse can include deceiving an elder loved one to donate money to fake charities. They may also be susceptible to fraudulent investments and scams. The world today is the con artist’s playground, and we must shut it down.

          What to Do If You Suspect Elder Abuse

          Now that we’ve uncovered some common signs of elder abuse, it’s time to address what to do about it. As mentioned earlier, only 4% of suspected elder abuse cases actually get reported. 

          Don’t let that happen – or keep happening – to your loved one!

          If you suspect that your loved one is experiencing abuse, it’s crucial to take immediate action for their safety and well-being. To honor our aging family and friends, we must not only take time to notice if they’re in danger, but more importantly act and advocate on their behalf. 

          Here are some steps to follow:

          1. Talk to Your Loved One: Be the Trusted Confidant

          Have a private conversation with the individual to express your concerns. Let them know that you care about their well-being and that you are there to support them. Ask open-ended questions to encourage them to share their experiences and feelings. 

          Be gentle, but persistent — it’s a delicate balance, but your empathy can open doors.

          2. Document Your Observations: The Detective’s Notebook

          Keep a detailed record of any signs or incidents of abuse you observe. Include dates, times, descriptions of the signs, and any conversations you have with your loved one or other individuals involved. 

          This documentation will be crucial if you need to escalate the issue – perhaps in a court of law. Which segues nicely to our next point.

          3. Report the Abuse: Sound the Alarm

          Depending on the severity of the abuse, you may need to contact local authorities, such as Adult Protective Services (APS) or law enforcement. Provide them with your documentation and any additional information they may need to investigate the situation. 

          Remember, you’re their advocate. Don’t hesitate to take action when your gut says to!

          4. Seek Professional Help: Call in the Cavalry

          But if you need clarity before taking action, consult with health care professionals, social workers, Board-certified Patient Advocates (like myself) and/or elder law attorneys to get guidance on the best course of action.             As with anything in life, there’ll always be good and bad days as a care partner. Sometimes, everything rolls smoothly. Your mom eats all her meals. The fog clears to offer clarity for your brother living with dementia. You’re completely prepared for an afternoon of doctor visits.

            Other days are a nightmare. Nothing goes right.  (Believe me, I’ve been there.) Of course, none of us is perfect – especially at demanding caregiver roles that require boatloads of dedication, empathy and resilience. 

            Every care partner makes mistakes. Amid the daily trials, it’s easy to overlook the importance of self-reflection. In this blog, we take a look at 7 key self-reflection tips for caregivers to improve skills, better prepare and manage self-care.

            Taking time to reflect on experiences provides valuable insights and helps us grow as care partners. But self-reflection itself takes some practice to assess realities and adjust routines accordingly.

            Begin by revisiting specific caregiving scenarios you’ve encountered. Think about a recent challenging situation, such as a disagreement with another member of the care-partner team or an incident when your loved one’s safety was in jeopardy. 

            Take a minute (and a few deep breaths) and reflect on the following:

            • What happened in detail?
            • Who was involved?
            • What could’ve been done differently?
            • How did it resolve?

            Caregiving problems often seem insurmountable – until we break them down into easily digestible parts. Analyzing the incident allows us to understand it more thoroughly, revealing patterns and areas for future improvement.

            Handling challenges as a care partner requires mental toughness. But having strong emotions is justified. You must honor how you feel. You feel a certain way for a reason, and processing is key to adjusting.

            Reflect on how you felt during a specific incident or over the course of time. Were you frustrated because your elder loved one missed an important medication? Has it been stressful coordinating care among their various medical providers? 

            Sometimes it’s not the caregiving disaster itself, but uncertainty or the perceived loss of control. Recognizing emotional responses allows us to approach future issues with greater emotional control.

            Self-reflection is like exercise. We get stronger with each repetition. 

            Spend a few minutes each day reflecting on your experiences. I find it helpful to write a daily journal documenting thoughts and insights. Even jotting seemingly incoherent thoughts can provide clarity in the long run. Reflect during quiet moments, like the work commute or before bed.

            Build a habit of mindfulness and intentional growth.

            Notice I said, “continual” versus “continuous.” It’s impossible to improve as a caregiver continuously – or uninterrupted. There WILL be more mistakes. There WILL be stagnant periods and even regressions in skills and energy. But regular self-reflection is a powerful tool for adjusting and making incremental improvements over time.

            Summer Heat: 9 Elder Care Tips to Keep Our Loved Ones Cool & Safe

              Heat is just a way of life in the Clearwater, FL area. But just because it’s common for stifling heat and humidity here – even in winter – doesn’t mean older adults are any more acclimated to it.

              We’re heading into the thick of summer now. The “dog days,” if you will. Even kids and younger adults step outside and break into a sweat immediately. So here are 9 elder care tips to keep our loved ones cool and safe in the sweltering summer heat. 

              Elders Typically Have a Harder Time Keeping Cool in Summer

              That’s for several different reasons. As I’m sure we can all agree, age itself tends to make us weaker against the elements. But older adults also contend with:

              • Chronic medical conditions
              • Inability to adjust to sudden temperature changes
              • Prescription medications affecting their ability to naturally regulate body temperature

              Yes, even the heat is something worth factoring into your loved one’s life-care plan. 

              1. Hydration is Key

              Any doctor will tell you to stay hydrated even in comfortable conditions. But it’s especially important when it comes to surviving the heat. It’s fundamental to preventing heat stress – and heat stroke. 

              Encourage your elder loved one(s) to drink water regularly throughout the day. The National Council on Aging advises seniors to drink one-third of their body weight in fluid ounces daily. For instance, a 150-pound individual should aim for 50 ounces of water each day. Incorporating hydrating foods like watermelon, cucumbers, and oranges can also help maintain hydration levels.

              Other medical experts recommend MORE FLUID INTAKE – anywhere from one-half to two-thirds of total body weight in fluid ounces. My advice to my Patient Advocacy clients, especially when it comes to contending with triple-digit temps, is to drink as much water as possible.

              2. Utilize Air-Conditioning

              Sometimes it’s best just to stay indoors. If our elder loved ones don’t absolutely have to be out in the heat, they should enjoy a refreshing drink in the cool air conditioning.

              If their home or other living environment lacks air-conditioning, some common public places with AC include libraries, malls and senior centers. Churches and other religious centers often offer cool places and activities to enjoy during extreme heat.

              3. Choose the Right Clothing

              Accessories such as wide-brimmed hats and sunglasses not only provide shade, but also protect against harmful UV rays, ensuring comfort and safety outdoors.

              As I mentioned earlier, many older adults take medications that can affect their body’s ability to regulate temperature. Blood pressure medications, certain antidepressants and others may cause difficulties – or even medical emergencies – in extreme heat.

              Review all medications with a healthcare provider to understand any potential side effects related to heat exposure. Adjusting routines or medications as needed can limit risks.

              5. Stay Informed & Prepared

              As with anything else in life, it’s important to be aware of your surroundings and inclement weather conditions. The last thing you need is to be unaware of a heat index warning on a family picnic with an elder loved one.

              Being weather-aware is vital during heat waves. Seniors and their care partners should stay informed about the weather forecast and heat advisories. Recognizing the signs of heat-related illnesses is crucial. Immediate remedies include moving to a cool place, hydrating, and taking a cool shower or sponge bath.

              Which segues nicely to the next point.

              6. Plan Activities Wisely

              Here in the Tampa, FL area, we obviously know the weather gets super hot. But that doesn’t mean we can’t honor our elder loved ones by sensibly timing activities to keep them active in family life. Schedule outdoor activities during the cooler parts of the day, such as early morning or late evening. 

              Avoid strenuous activities during peak heat hours. Gentle exercises like walking or gardening can be enjoyed safely when planned around the day’s weather conditions.

              7. Apply Sunscreen Regularly

              “WEAR SUNSCREEN!” People of all ages should always wear sunblock, as the positive anti-cancer and anti-aging effects have been well-documented. 

              Protecting the skin from sunburn is vital, especially for our elder loved ones whose skin is more sensitive to the sun’s harmful effects. Seniors should use a broad-spectrum sunscreen with an SPF of 30 or higher, applied at least 15-20 minutes before going outside. Reapply every two hours – or more frequently if sweating or swimming – to ensure continuous protection from deadly UV rays.

              8. Create a Cool Environment

              Keeping the living space cool is essential. Use AC units and fans, and keep curtains or blinds closed during the day to block out the sun. In the evening, when it’s cooler, open windows to allow for ventilation. 

              Even if we believe our loved ones are comfortable inside, a single fan often isn’t enough. AC units also may fail. Monitoring indoor temperatures can help maintain a comfortable environment. Thankfully, technology today like cloud-based climate controls and medic alert systems can keep care partners updated about indoor conditions in real time.

              9. Nurture a Support Network

              From decades of life-care planning experience, I’ve seen firsthand the value of a strong support network. Ensure elder loved ones have regular check-ins with family, friends or neighbors during extreme heat.                     ‘The Joy of Dementia (You Gotta Be Kidding!) – Creating a New Performance of Dementia…and Life’ 

                    Thursday, July 28th – 3-5 pm

                    I’m proud to join coordinator Mary Fridley and member Nancy Nelson of the Reimagining Dementia Coalition as we reverse the “tragedy narrative” and stigma around dementia. We will address the growing demand for more innovative, relational, improvisational, playful and humanizing approaches to dementia care. There’s ample opportunity to embrace community, creativity, joy and growth for persons living with dementia and care partners.

                    This will be an experiential workshop highlighting:

                    • Improvisational games
                    • Creative exercises
                    • Philosophical/performed conversation 

                    All of which aim to foster a supportive environment to enjoy the “non-knowing growing” that’s possible, even in uncertain situations. As I often say, “I may have Alzheimer’s, but Alzheimer’s doesn’t have me.” Yes, we can still find joy with dementia.

                    Persons Living with Dementia Promoting Wellbeing Through Education and Advocacy

                    Let’s have a meeting of the minds – dementia minds! (And, our marketing director will be happy to know there’s a dog involved.) I’ll be joined by some fellow co-founders of the National Council of Dementia Minds, Mark and Brenda Roberts (and their cute service dog, Sophie), to offer first-hand perspectives on living with dementia.

                    Each of us has dementia. That’s what prompted us and our other awesome co-founders to launch NCDM in 2019. We’ve learned to cope and build a life worth living in our own special ways, and we intend to share those daily realities. We’ll also address the many ways we provide education and advocacy in our respective communities and spheres of influence.

                    And once again, we’ll share a few laughs with a funny video and undoubtedly some witty banter during the Q&A portion.

                    Death Doulas: Compassionate End of Life Care

                      Death doulas deliver compassionate care at end of life. They’re non-medical professionals known by many names: death coaches, soul midwives, transition guides and end-of-life coaches, among others. Much like birthing doulas ensure care and support to families as babies say hello to the world, death doulas provide holistic care and comfort as elders say goodbye.

                      We expect the same honor and dignity in death as we do in life. Many elder care partners seek comfort care and support for their loved ones in their final days. They often turn to hospice care agencies to provide and coordinate a plethora of services to help the dying individual and their care-partner team.

                      But what exactly do death doulas do? Will hospice help to coordinate such services? Are they a good fit within your life care plan? For so many, the answer is yes.

                      Death Doulas in Your Life Care Plan

                      A death doula, much like a midwife during the days and weeks around the birthing process, attends to all aspects of care and support during the dying process. You may find a death doula holding a dying person’s hand, listening to family stories or even washing dishes so stressed care partners may rest.

                      Their loving care involves so much more before, during and after death.

                      Life Care Planning

                      You may choose to include a death doula in life care team building and planning. They are trained and often certified to serve numerous vital roles in administering and advising, among others:

                      • Pain management
                      • Daily responsibilities
                      • Spiritual care
                      • Emotional counseling
                      • Home environment
                      • Respite support
                      • Honorary services & burial

                      A death doula will work alongside hospice and other care partners to understand your individual needs. They may also help to educate the care-partner team about realities of the dying process. With greater understanding comes better planning.

                      Companionship

                      Most elders in their final days want someone at their side. It’s easy to understand this very natural human desire. The dying process, for many, includes lifelong self-reflection, flipping through a treasure trove of memories and experiences. They often like to tell these tales — good and bad, love and loss, children and grandchildren, school and career.

                      Death doulas may join family and friends in hearing hours of fascinating stories. They may also sit alone in silence with your loved one, both enjoying the pleasant simplicity of the other’s company. These moments of companionship, conversation and silent contemplation bring comfort to everyone involved.

                      Emotional Counseling & Spiritual Care

                      The dying process can be overwhelming and scary for everyone involved. A death doula is an expert at counseling and coordinating spiritual care of any denomination. 

                      A person at end of life may experience a spectrum of emotions from extreme agitation and sadness during pain to elation during a visit from close kin. Family and friends experience overwhelming grief for much of the dying process, often with very few happy moments or glimmers of hope. Religious and emotional counseling can make sense of intense emotions and allow everyone to say goodbye in peace and without regret.  

                      Respite Care

                      Everyone needs a break, a breather, some time to themselves or to run personal errands. That’s why you’re just as likely to spot a death doula at your elder’s bedside as running a vacuum in the living room. Doulas support you in any way necessary — whether watching children so adults may visit in peace or reading a book with your elder as you run to the store. 

                      Team Coordination

                      The dying person and their care partner team each have important roles to play. A brother may cook dinners while a daughter or niece may coordinate bath times or daily meds. The death doula can be a traffic signal for the team, reminding care partners to perform tasks or stepping in themselves for support.

                      Some with severe terminal illnesses — like cancer or dementia — prefer a relatively small group of close family and friends to visit their bedside. Your doula can coordinate visitations and facilitate an honorable environment for those wishing to pay respects. 

                      Honorary Services

                      Some elders and families prefer for services in the home or care community. If desired, home services provide a private, dignified environment to honor life. Home vigils are common for:

                      • Grief management
                      • Religious preferences
                      • Natural burials

                      Services may be religious or spiritual and also known by many names. Some last hours or several several days. Or they may be simple celebrations of life. Families may decorate caskets and recall life stories in the presence of the deceased. Your death doula can coordinate bedside services to include prayers, hymns, scripture readings and other rituals dear to your faith and traditions. 

                      Is a Death Doula the Right Fit for Your Care Partner Team?

                      It’s a matter of preference. Death doulas can serve roles in all facets of end-of-life care. Just as a midwife would assist the expectant mother, baby and family before, during and after birth, a death doula provides caring structure, support and guidance before, during and after death.

                      Don’t wait too long to craft a plan and build a care team. Your loved one likely has specific needs and wants for their final days. You’ll only learn these through ongoing conversations while your loved one is cognitively aware and care partner emotions are calm enough to assess all options and wishes.
